REMINDER: Spring Field Trip Fun is right around the corner:
SPRING Grade Level Field Trip Information & Permission Slips have been sent home. Each student was provided a packet with a permission slip to attend the field trip as well as guest background check information.
ALL guests who plan to attend a field trip are required to complete all background check documents. This must be completed for each guest attending. If more forms are needed please contact your child’s teacher or PHES office. Field trips are quickly approaching. Please complete the background forms as soon as possible, these do take some time to be approved through the Missouri Department of Health and Senior Services. See packet for more details.
ONLY guests with approved background checks will be allowed to attend a field trip. If your background check is not complete and approved you will not be allowed to attend.
Please understand it is ALWAYS our top priority to keep all students safe. We appreciate your cooperation.

1. Pleasant Hope School District FERPA Form (Complete paper form & Return to PHES classroom teacher)
2. Worker Registration (Complete online at www.health.mo.gov/safety/fcsr One-time registration fee of $14.00 - pay online)
3. Missouri Department of Health and Senior Services Family Care Safety Registry Volunteer Background Screening
This gives PHR6 District Office staff permission to look up the Worker Registration through Missouri Department of Health and Senior Services Family Care Safety Registry. (Return to PHR6 District Office)
